As a registered Medicare / DVA provider, we accept referrals under the Chronic Disease Management Plan (10958) for Medicare and an appropriate referral form for DVA. Please note that a Gap Fee is payable for Medicare.

We currently service Plan Managed and Self Managed NDIS participants only.

Please check with your private health fund provider if you are covered for occupational therapy prior to making a booking.
